Liability Waiver Agreement

In consideration of being allowed to participate in the Lynchburg Area Youth Sports Summer Play Day, I acknowledge and agree that: 

1. I understand that the sports my child will be playing throughout the day may involve strenuous physical exertion and activities and I am fully aware of the inherent risks and dangers, including but not limited to physical illness, injury, or other consequences arising from my child's participation in the tournament. I agree to assume full responsibility for my child's health and safety and for all risks associated in their participation.
2. I understand that Lynchburg Area Youth Sports and the University of Lynchburg are not responsible for any lost, stolen, or damaged valuables or property.
3. I expressly waive, release and discharge Lynchburg Area Youth Sports, University of Lynchburg and their respective volunteers, coaches and representatives from any and all claims, demands, causes of action, damages or suits at law and equity of any kind, including but not limited to claims for personal injury, property damage, medical expenses, loss of services, on account of or in any way related to or growing out of my child's presence or involvement at the Summer Play Day event.

By checking the below check box, I acknowledge that it will be accepted as my legal signature, I certify that I am the parent or legal guardian of the child being registered, and I agree to the above terms of this Liability Waiver Agreement in full.
